Could you please try removing MailChimp API key from Enfold theme options and using only the plugin and check if that helps? If it does not, please try contacting plugin authors as the issue does not seem to be Enfold related :)

Please go to Enfold theme options > Header and choose to display “Logo left, menu below” option and then enable “Sticky” and “Shrinking header” options in Enfold theme options > Header > Header Behaviour tab. Then add following code to bottom of Enfold theme options > General Styling > Quick CSS tab

FTP logins did not work for me. Could you please check them once again?

Edit: I updated your header.php file in Appearance > Editor and that helped. You only had Facebook domain verification code in header.php file. I moved it to the bottom of functions.php file instead. You can remove header.php file from your child theme.

I could reproduce the issue on your page. Submitting the form throws an error with “Internal Server Error” status.

I do not think the issue is related to Enfold but to make sure that is the case, we would need to test further. Could you please create a staging site and post temporary FTP and WP admin logins here privately so we can work on your staging site instead of the live one?
