Please test now. I added the following to quick css:
#mc-embedded-subscribe{ width:200px!important; color:#fff!important; text-align:center!important; }It should be better now, I believe there was some sort of css conflict.

I added the following to quick css and it seems to have fixed the issue:
@media only screen and (max-width: 767px) { #mobile-advanced .avia-menu-text{ margin-left:5px!important; font-size:13px!important; } #mobile-advanced .menu-item a{ line-height:75px!important; } }Let me know if this is okay.

Okay and if you need it to look like the first image you pasted above add this to the css:
#FormField_EmailAddress.text_input { margin:0 10px!important; } #FormSubmit.button{ margin:0 20px!important; }Let me know if this works and then we can work on the responsive end.

You can use the browser element inspector to view the form styles. However, as I stated be sure to add a unique class to the element that your are adding your email script to. This is just to add a little insurance that it won’t conflict with the sending of the form.

Strange. I also tested with a youtube link and encountered the same issue. If you don’t mind, and are comfortable to do so, please view the following thread and attempt the solution presented. How did you add the classes onto the form? You can add a class to the section you added your e-mail script to (code box or text box?), and then target and style the fields from there.

I am not able to see the theme files from the dashboard (header.php, etc.). Is it okay if I deactivate your active plugins in an attempt to view those files?

1) No, those particular errors should not cause an issue unless you were intending to use the referenced layerslider skins.
2) No, they should not.
3) Errors can display for a number of reasons when completing any WordPress install. The majority of them I have seen tend to be no fatal, and could just be based on missing files, server configurations, etc.
