Well, the link with credentials expired. I could not check. Are you using some kind of plugin to have different menus? If yes, you might want to check the plugin docs or write to the author and ask him how to make a menu item appear on all menus.

Selecting a parent page does not affect permalinks.

To make the custom structure like you want you might want to consider a plugin or hire a freelancer to do it for you.

It comes either from some plugin or your hosting provider or another website in your configuration. It does not come from Enfold and you will not find it in footer.php.
For now you can hide it like this
