Please deactivate all plugins one by one to check which one is causing this issue and let us know if this solves the problem.
Also, you can comment out the custom function fade_out_on_scroll in your functions.php and test without it.

Unfortunately, it would require quite some time and customization of the theme to achieve this, so I am sorry to tell you that this is not covered by our support. However, if it’s really important for you to get this done, you can always hire a freelancer to do the job for you :)

It does not work on some hostings and so we cannot really fix it. We offer the Smash Ballon plugin integration instead.

The Enfold menu is the one that shows pages and the custom menu item is not a page so it will not show up in that auto-generated menu, but you can build your own menu and show it in the sidebar. Do you need help with that?

With the way, the HTML layout is, the border will not stay. The layout that you need should be custom-built, it can be difficult to do with the theme.
