There is a known issue with the pagination, which is been planed to be fixed on the next update.
Please till then, check: https://core.trac.wordpress.org/ticket/35344#comment:34
for a quick fix.Also, Please do not post in multiple topcis the same issue, we are handling all of them as much as possible ( @schleeh )

You will need to code the form, to be working as responsive. You will have to use media queries to make it happen, else if that is not possible from you, you will have to hire someone to do it for you.

It is strange that the plugin is having that issue with enfold. As it seems the plugin author is really active, so could you please drop him the question and let us know what the results would be?
Else, you will have to use a different plugin from the repo, it seems that there are a lot
