Multiple answers for checkbox

Hello,

I need to make a question on my contact form that has the option to have multiple answers.

For example the question needs to be: “Please check the following fabrics you wear or use.”

And I need a checkbox to the left of each answer:

Polyester, Acrylic Fabrics, Rayon, Acetate and Triacetate, Nylon, Static Resistant, Stain Resistant, Permanent Press, Wrinkle-Free, Stain Proof.

Would you please help with the code to accomplish this?
