    I was wondering how do I insert a description above checkboxes please.

    I have a website that I’m working on and the form requires to have the following fields

    What service do you require?
    [ ] Carpet Cleaning
    [ ] Upholstery Cleaning
    [ ] Tile / Stone / Marble Floor Cleaning
    [ ] Driveway / Pavement Cleaning

    However, when I select the checkbox functionality, there is nowhere for me to add the label above the fields, which in this instance would be “What services do you require”

    As the visitor needs to be able to select a number of services, I am not able to use the “Select Element” function.

    You’d need to use a specialized form plugin for that, like:
    http://wordpress.org/plugins/contact-form-7/

    Or GF:
    Gravity Forms
