I tried it – it works…..

Sorry, my fault – it really does not work (checkbox no 7)

maybe because of the chinese characters?

Hi!

Somehow two check boxes have the exact same input name. You should be able to either delete the form and re-create it or maybe remove the checkboxes and re-add them in to fix it.
