I don’t believe this is possible out of the box, and you would likely need quite a bit of custom css to achieve such a layout and make it responsive.

Again, this is also a function that is implemented within the map script itself. It’s not a function of the theme.

This may be a compatibility issue between v.4.2 and the plugin itself. I’m not quite sure what could be the exact cause and will like likely have to report this to the devs before a fix is found. Just to be sure, if you are able to set up a staging install of 4.2 with the plugin please test to see if the issue continues.

#2 Again, I’m still unclear as to exactly what you mean with this.
#3 Which section do you consider the menu and header. I believe figuring your interpretation of this will clear up the other 2 as well.
