Thanks for the quick reply, Rikard! Took me a few days to get back to trouble shoot this.
I reinstalled the theme.. wasn’t the problem.
I thought I had tried disabling all the plugins, but apparently I left a few activated…
The issue ended being caused by the Wp-D3 plugin… once disabled the triplicate image problem stopped.
