That is a custom form and not included to the theme.
You will need some custom CSS to use it.

You will need to either run an update to Facebook meta through their API or try to check it how manually what image is been added.

You can import our demos, but combine, means that you can copy / make them like that as our ALB is builded on all themes
so they can be combined.

You can build it by copying the results based on the ALB.

It seems that there are way more pages on the menu, that it should be.
You must edit it from Appearance -> Menu and that will do solve your issue.

2. You can change the size, but I can not login with the informations you provided us
