    Please add following code to Quick CSS in Enfold theme options under General Styling tab to fix header issue

    .avia-safari #header { background-color: white; }

    I could not reproduce search icon issue and could not find your form. Can you please post the link to your page where we can see the forms?

    regarding icons is “social icons” not search icon ;)
    and you can find the forms in:
    http://www.luqapharma.com/newsite/contact-us/

    when using Safari 5.1.7 (from mac os x snow leopard 10.7.5)
    the box in the forms and the social icons dissappear

    @Andy     checked your site and on Safari 9.0 on OSX 10.9.5 and said it looks fine for him as well. Can you please try updating Safari while using 10.7.5 and check if that helps?
