Splitting form details & design alterations

Hey there,

How can I split forms elements across a page so that NAME, ADDRESS & SUBJECT are down the left column but the INFO BOX & SUBMIT BUTTON are down a right column? Is this possible with this theme or do I have to look at an advance form builder?

also:

How do I alter the following elements for just a contacts page form on my CONTACT ME page:

1) How do I change the background colors of the boxes?

2) How do I change the padding between boxes?

3) How do I change the corner radius and add soft shadows (with opacity control) on boxes?
