    Hello.

    I have added a contact form to my page and would like to add a box with several options. When I create a box within the form, it appears (as you can see in link) on left side, but not in demo on right side. It does not appear on livesite either. I published the page, and suddenly my form was gone in front end, and change to default in backend.

    I dont understand. Do you see a field with select element within the form?
    I added wrong screenshot, in private field is new and correct screenshot of problem.

    You need to add a select element field to the form and see how it behaves when you do this.

    We added a “select” element in the contact form. It is working. Please remove browser cache or hard refresh before checking the page.
