Please reply to my question! Contact form not working

I have posted this before, but I think it is lost in all the messages, but I am desperate at the moment.

A client of mine has a form on this page:

The form is not working. She says she has about 40 people that filled in the form, but she hasn’t had any e-mails about it. Is there any way to:
1. fix the form
2. find out who filled in the form, are they stored somewhere
I desperately need to know, as she is blaming me of losing income now. :(

I think my question is lost in the other request, but my client would really like to know today if the e-mail addresses of people that filled in the form are lost or if they are to be found somewhere.

Could you please let me know today? (I normally don’t put a time scale on things here, but I am desperate and my client is VERY angry :()

I would like to replace the form with a contact7 form, but I don’t know if I’m then interfering with your work. So I just placed a warning for visitors on it for now.
