Mailchimp – Form fully filled, it say isn't filled

I followed your guide to create a form. I created an API keys in mailchimp, I checked that it works (and it works).
I created a mailchimp form, but when I try to test, if i push subscribe, it say that the form isn’t well-filled (I tried to translate it as well as I can).
