I created forms with required fields. In preview mode (in WordPress) everything works well: when I click ‘send” the browser returns a message that some fields are required and indicates the fields. When I test the form live nothing happens: when I click “Send” the browser just returns an empty form again and again without any message.
I use the plugins: Gravity Forms 1.8.22, Gravity Forms Survey Add-on 2.4 and Gravity Forms (Nl). WordPress version 4.1

That’s quite a big form you have there. I didn’t see any errors or anything unusual in the source code. What happens in the WordPress default theme? Have you contacted gravity forms support about this?
Is it just this form that is having the problem?
Make sure to deactivate all of your other plugins except gravity forms while testing.

I deactivated all the plugins and activated them one by one. I found out that the problem occurs when activating the plugin Nelio A/B Testing.
I will contact them.
