Well, the easiest way is to make the whole table with the html and style it as you need.

How can I change dropdown values of other select box depending on selection of another dropdown select value in wpforms.
example:

“Select MBBS Universities Office” > then the value changes in “Select MBBS Universities Branch”

I’m not sure if it’s possible to add a contact form to a layerslider, but you can try to copy the shortcodes of a form if you enable debug mode: https://kriesi.at/documentation/enfold/intro-to-advanced-layout-builder/#debug-mode
