URGENT! Our forms have stopped working properly. This appears to have happened before the latest update. I just updated in hopes that the problem would be resolved, but it did not work.
Where the form title is suppose to be, we get a ” mark.
Also, the redirect to new page does not happen and the email does not get sent. Some examples of this are at-

Can you please try updating WordPress to the latest version 4.0.1 as well?

The issue started to happen when you update the theme to Enfold 3.0.4 and leave WordPress to version 4.0. Glad it is working now.

Same Problem on our customer site (no running on my test-site: http://kultur-design.com/news/news-abonnieren/ )
Just the captcha is showing up – no form.I already deactivated the three plugins and running WP 4.0.1
any ideas, any suggestions what to do?

sorry for disturbing – updating the enfold theme to 3.0.4 resolved the contact form problem – everythings fine with that great theme – thanks to kriesi and your team
