Editing Forms

Hello,

I need to create a simple form with some text inputs and checkboxes, but I need to be able to add a subhead in between the fields and the checkboxes.
I can’t see a way to add any html elements between form elements in the editor.
This is the form I need to replicate: http://www.johnlaferla.com/volunteer

And the part i’m asking about is how to add “I am Interested in Helping with the Following” before the checkboxes.
