Deactivate Mailchimp pop up form code from website

Hi! I need to change the email marketing provider form Mailchimp to Milerlite.
So I have a Mailchimp pop up form embedded on the website that I need to remove.
I remember asking for your help to add that code to the website and now I should need your help again to delete it :)
The pop up script should be something like:

I cannot remember if some plugin was used. Anyways, would it be possibile for you to delete it?
I am enclosing my website login credentials in the private area.
