Hi,
I suppose this issue has to do with the WP update. My running sites all got the same problem; not showing the elements of the form. Only title and submit button.
This is what’s going on when checking in the browser, I see this happening: http://imgur.com/HEs4nErHow do I solve this?

Please make sure that you are using the latest version of Enfold which is 3.0.4 – http://kriesi.at/documentation/enfold/updating-your-theme-files/
