Have tried deleting the form and starting again but same issues persist. Form Title, Hide Form Labels and Theme colour scheme all revert back to default when I make changes elsewhere on the page. Very frustrating as it changes the page layout.
I have the most up to date version of Enfold and WordPress installed.
The mailchimp form element seems to work okay which I will use for the time being. I am guessing if I want to use another auto-responder I will have to use another plugin like CF7?

please update to Enfold v4.0.2.
If that doesn’t help, then deactivate all plugins you’re using, to check if one is causing this issue.
Also there might be some corrupted files, so please delete all theme files completely via FTP, before installing a fresh copy from your themeforest account. Here is a short tutorial on how to install the theme via FTP, in case you are not sure how that works:

AndyI experience the same problem. To deactivate all plugins (except Theme Test Drive) and perform a hard refresh has not solved the problem. Please take a closer look.

Thanks for the login details. Sometimes it helps to recreate the form on a new page, could you try that please? You could also try to activate debug mode in order to see builder shortcodes: http://kriesi.at/documentation/enfold/enable-advanced-layout-builder-debug/, and paste the shortcode from the old page to a new one.
