Contact Form Not Showing

Hey Guys!

So I put the Contact Form in and update the page and it only shows the “Submit” button. The form fields are there but their not showing up. Why?
I looked at the code and here it is

I can’t show the website to you because I have it down currently for edits and don’t want to launch it yet.
