For some reason it’s only displaying the bottom of the form. http://dentists.theimplantexperts.com/referral-form/ There should be 10-12 entries. They are there in the backend.
The form on the contact page http://dentists.theimplantexperts.com/contact/ also isn’t displaying properly.
