Hi, I am using the enfold theme and I would like to have a contact form in the header/slider like the link below.

Can you please let me know how to do that??

If you enable debug mode in order to see shortcodes then you could copy the contact form shortcode to where you would like to add it: http://kriesi.at/documentation/enfold/enable-advanced-layout-builder-debug/
You could try adding a widget area to the header: http://kriesi.at/documentation/enfold/adding-a-widget-area-to-the-header/
