Advanced Custum Fields displayed on a specific page using AVIA

Dear Ladies and Gentlemen,
I’m using Enfold for my website and i’m quite happy with enfold.
But today I need your help.
I want to display some costum fields an a specific page type.
How to install an new pagetemplate in wordpress similar like Standardtemplate, Archives and Blank? I would like to design a custom one.
In addition how to display the costum fields? I tried it with “echo get_post_meta( $the_id, ‘mitarbeiter_vorname’, true );” in loop-index.php. But there are two problems. No1: if I’m using the AVIA will not be displayed. It will only displayed when I use the standard editor.
Problem No2: It will be displayed in blog post only, not in page.
Could you help me please?
