I bought https://elements.envato.com/hospital-doctor-directory-WNCS8L3 and i activate the plug in (there is no theme)
But do i have to purchase this beside plug in

Yes, Enfold theme needs to be purchased separately. It seems like plugin authors have used Enfold theme and based their demo on Medical demo – https://kriesi.at/themes/enfold-medical/ which can be imported with a single click within a few minutes :)
