The problem is now resolved, there was an issue with w3 total cache specifically the css minify setting needed to be disabled.

No doesnt look like any styling plugins are being used, please take a look and let me know the issue thanks.

No as no new plugins have been added since the problem started. The problem seems to have started with the last template update.

Yes see links, the icons seem to be disregarding the settings on the admin.
