Hi,
Outside of contacting the hosts, you could try Gravity Forms or Ninja Forms. They both work well with Enfold. Although if this turns out to be a hosting problem they may have issues delivering as well.

This is possibly something that may need to be addressed with your host also. Some web hosts block PHP web forms unless requested otherwise. I know you mentioned they may have been working before but you want to make sure no hosting changes were made.

You have to add a fallback image because the video is disabled on mobile device. The workaround is to hide the slider on mobile then display the actual video. Use css media queries to toggle the display of the elements. Example here; https://kriesi.at/support/topic/home-page-video-on-mobile-phone/#post-449787
