I’m having similar issue. I can upload images to library, but they will not insert to a page or post. I had someone contact with with the same issue on a different theme. The only common element is they both use the same visual editor.

Just wondering if there is an issue with the visual editor?

I did deactivate the visual editor on the page, and it did not solve the issue.

Any ideas?
