I figured out how to make it work. But couple of things has been have to change on my code setup.
When the form is added to the page via it’s own custom field section on edit product, the form structure becomes different than the original. ( I can’t figure it out why, i also write to plugin author about this.)
But when i add the form to the product page via shortcode, it’s structure is completely right and dependency fields are working. But lots of setting and coding i’ve done has been a waste because of they were designed for the previous settings.

If you still may help about why the form structure is being different when added by plugins custom field, it may help for future.
