This is kind of odd because the form was working just fine last week.
Turns out that if conditional logic is used on the form it does NOT show up. Once conditional logic is used, the form displays correctly. According to GF’s website it is because the footer is not being called thus creating the problem.

How can this issue be resolved? I’d like to continue using Enfold, but if it won’t work with GF it isn’t going to work for my website. :(
I have an issue with Gravity Forms as well. It works fine on one page that has a simple 3 field form. My other form which is longer doesn’t even show up. It shows fine if I switch to a different theme.
I see what I did! I didn’t extract the original file I downloaded to get to just the zipped theme folder.
