Please add following code to Quick CSS in Enfold theme options under General Styling tab

    display: none;
}
.home .avia_ajax_form .form_element_fourth {
    width: 19%;
}
.home .avia_ajax_form p {
    width: 19%;
    clear: inherit;
    margin-left: 10px;
}
