My Contact Form shows header (send us mail) and the Send button, but no fields. I have now taken an unaltered Contact Form just to see if I had made any mistakes, and it doesn’t show either. I have deactivated all plugs-ins but that made no difference (they are now reactivated).
I have had to remove the contact form and enter e-mail addresses instead until I get this resolved, so you can no longer see an example of the problem. I can reproduce it at anytime if needed.
