Contact Form fields suddenly gone

Hi there,

My client just contacted me today to let me know that his contact form ‘disappeared’ from his contact page.

I logged in to WordPress and looked at the information, added new contact form fields and am unable to see any of the fields (other than the submit button) on the page.

Any suggestions for how to resolve this issue?
